Instructions for using the PASS Form

These PASS forms use the Microsoft Word 2000 format. They must be used with Microsoft Word 97 or a later version in a Microsoft Windows environment.

These forms are similar to 545 PASS forms, but are not the official SSA PASS Form. However, they are an acceptable substitute.

There are two versions of the RCEP 7 PASS form:

  1. The plain PASS Form
  2. The annotated PASS Form, with helpful comments or suggestions from the Region 7 PASS Cadre about items that have given some people trouble in the past. These comments are inserted in the form below each question.

Once you have selected a file and downloaded it, you will only be able to fill out the gray shaded boxes in the form. Your "tab" button moves the mouse from one gray area to the next. Some boxes are text fields into which you type your information or answers. Other boxes are check boxes - for which you press the X button or click with your mouse to check or uncheck.

The plain Pass Form has some special "help keys" embedded in it (curtesy of the Rural Disability Institute at the University of Montana). These are hidden helpful comments linked to some of the gray shaded boxes. You access the information by pressing the F1 key when your cursor is in a particular box. There is an instruction sheet describing this and a few more details.

We recommend that you use the "save as" function to save the PASS Form under a new name each time you fill out the form so you don't write over the original, blank version. You may want to save it as the name of the person for whom the PASS will be written.
