 Timeline of Tony's Story

June 26, 1993 - surgery, atlanto-axial stabilization, California Medical Center, San Diego, California

Fall 93 to November 8, 1995 - Volunteers at Veterinarian's clinic. Does some cleaning and plays with the animals.

June, 1995 to August 1995 - Volunteers @ retirement home (taking water to residents, helping with activities)

November, 1995 to June, 1996 - works part time at Pets-R-Us on weekends, helping clean up the store and take care of the animals

August 27, 1996 - The start of Tony's Senior Year. IEP review. IEP is amended. Meeting is attended by school staff & case manager.

September 12, 1996 - As part of his work study program at school, Tony begins volunteering at 3 jobs - janitorial work at a textbook warehouse, shelving at the library, and stocking at the foodbank.

Story Begins

January 3, 1997 - Initial interview/Supported Employment tours. Sharon asks about college. Supported employment assessment is discussed.

January 10, 1997 - Sharon calls VRC-chosen Glenbrooke. School records still not in.

January 17, 1997 - Declared eligible.

February 7, 1997 - Meeting at HS between client, mother, VRC, and Glenbrooke to plan Supported Employment Assessment (SEA). SEA will run from March 1st to May 1st.

February 17, 1997 - Meeting at Glenbrooke (No ROC).

March 18, 1997 - Glenbrooke announces they won't start the SEA until close to graduation

April 9, 1997, April 10, 1997, April 14, 1997 - Series of calls about problems scheduling Glenbrooke.

May 1, 1997 - Kennelworth calls VRC because Glenbrooke's people have missed meetings.

May 19, 1997, May 27, 1997, June 3, 1997 - More calls about problems with Glenbrooke's scheduling.

June 9, 1997 - Trouble with Pet's-R-Us assessment. Allison got lost.

June 17, 1997 - Graduation from Kennelworth.

June 24, 1997 - Staffing meeting at Glenbrooke. Decide to proceed with supported employment services.

July 2, 1997 - Family has vacation plans for Florida until August 25th

July 7, 1997 - Tony's Grandmother dies, vacation plans cancelled.

July 20, 1997 - Tony's mother, Sharon, gets Tony a job at the City swimming pool through the City's summer jobs program.

August 2, 1997 - Glenbrooke job developer Suzanne Boller discusses the case with VRC.

August 19, 1997 - Tony's summer job with the City ends.

September 1, 1997 - SE services scheduled to start.

September 26, 1997 - Sharon calls VRC-upset because Glenbrooke has not been doing job development. Glenbrooke says they've done all they can.

October 9, 1997 - Sharon decides to change to Medallion

October 27, 1997 - Medallion contacts Pet's-R-Us about a job for Tony. Tony is not clear on what types of jobs he might like.

November 12-15, 1997 - Medallion conducts a supported Employment Assessment of Tony, at no charge, for jobs in food service and laundry.

December 5, 1997 - Working interview with Vegoni's, which goes well. Tony takes a job application home.

January 9, 1998 - Orientation at Vegoni's.

January 15, 1998 - IWRP Meeting held at Mid-City Bowl. Decide Tony's hours and that he will have a job coach through May.

January 17, 1998 - Tony begins working at Vegoni's. Laura is his job coach.

January 22, 1998 - Megan Stewart calls, conflict with other client's (Becky) hours being reduced. Central office said not illegal, but very disrespectful of Medallion.

January 26, 1998 - Natural Supports Plan written.

January 30, 1998 - Sharon calls VRC, hearing rumors about job problems with Becky.

February 2, 1998 - Mom is upset that job coach was changed, that shift might be changed to evenings. Coach problem is sorted out. Mom still upset about shift change.

April 17, 1998 - Tony, Sharon, Lillian Thoms (VRC), Tess Cooper (case mgr.), Laura Hayes (job coach), and Medallion program mgr. meet at Medallion to discuss his progress.

May 17-24, 1998 - Tony takes a week's vacation.

June 18, 1998 - letter to Tony, he is officially employed and the 90 day monitoring period begins

August 1, 1998 - Tony's job support funding is transferred from VR to Davis County Family Supports for long term support.

September 25, 1998 - Close file
