1971 - Rich has trouble with a teacher, unspecified traumatic incident
1981 - Rich graduates from high school. School counselor refers him to VR. He is diagnosed with bi-polar disorder
05/24/82 - VR sends Rich to Oklahoma Technical College to learn Dry Cleaning management
11/15/83 - Rich finishes Oklahoma Technical College program.
08/12/83 - Rich is unable to find a job in laundry, but finds a job as a stocker in a grocery store. His VR case is closed.
07/27/84 - Rich moves to University City
06/14/85 - Rich begins seeing a psychiatrist for depression.
02/14/88 - Rich begins working at Discount Mart
08/16/88 - Rich admits himself to University Mental Health Center
11/30/88 - Rich sees doctor at University Hospital. He is passing out in public places. continues to see doctor once or twice a month for next six months for pseudoseizures, narcolepsy, and allergy problems.
10/17/89 - Rich sees doctor about allergy problems. Continues to see doctor once or twice a month for the next year for allergy, hypertension, and occasional pseudoseizures.
11/15/90 - Rich's supervisor at work calls doctor because Rich has passed out in the store for 30 minutes. Later doctor's visit does not show any cause.
12/04/90 - Rich goes to ER at 10:15 pm for passing out. He had passed out at work again and three other times that day.
12/05/90 - Rich passes out waiting to see his doctor, but later wakes up. After an inconclusive medical exam, he is referred to counseling. Begins treatment for anxiety disorder.
06/02/91 - Rich admits himself to University Mental Health Center
08/21/92 - Rich has surgery for a hernia
11/18/92 - Rich leaves job at Discount Mart
02/17/93 - Rich begins 3 months of monthly medical checkups to monitor hypertension and possible goiter.
1993 - Surgery for hernia
1993 - Surgery for removal of a goiter (neck mass).
1993 - Rich admits himself to University Mental Health Center
06/01/93 - Rich begins recreational therapy to improve social skills. Discharged 12/13/93
02/10/94 - Rich begins working occasionally for Just In Time Temporary Services
02/28/94 - Rich begins working once a week for Nickel Saver advertising/coupon paper. The job ends 7/94
03/10/94 - Rich begins seeing psychiatrist at Temporary Services Program on a monthly basis.
03/26/94 - Rich participates in medical drug experiment through University Hospital. lasts two months.
05/10/94 - Rich admits himself to University Mental Health Center, afraid he may harm himself. released after two days.
05/16/94 - Rich goes to the emergency room for recurrence of problems passing out
06/09/94 - Rich has sleep study done because of problems passing out over last two months. No sleep problems are found.
08/16/94 - Rich begins working part-time at Hennigan's Department Store to pay off $2000 in credit card debt.
11/03/94 - Psychological evaluation
12/14/94 - Rich leaves his job at Hennigan's Department Store
8/28/95 - Rich works at Arthur's Grocery store for one week
09/05/95 - Rich calls VR to re-open case. Application forms mailed to him.
10/30/95 - Completed application forms received by VRC
11/14/95 - Initial meeting (NO ROC?) - release of information forms signed.
12/04/95 - Letter to Rich - medical information has not been sent yet, eligibility not established. Rich is asked to contact VRC within 2 weeks
01/19/96 - Letter to Rich - scheduling appointment for 2/8/96
02/08/96 - Rich meets VRC & casemanager. Medical records not available yet, so eligibility not established yet. New release forms signed and mailed.
04/30/96 - Letter to Rich, eligibility established
05/14/96 - Rich & Tim decide Rich will go to Medallion for voc assessment.
05/20/96 - voc eval at Medallion begins
05/31/96 - voc eval ends
07/11/96 - Rich & Tim review voc eval and recommendation for job readiness training. Tim writes comprehensive assessment for initial plan
07/15/96 - Rich & Tim review IWRP from readiness training & Rich signs it.
07/22/96 - Rich begins 4 week job readiness training
08/20/96 - Tim authorizes extension of Rich's job readiness training
10/03/96 - Tim authorizes further extension of Rich's job readiness training
11/15/96 - Medallion calls Tim to tell him Rich has found a job
11/18/96 - Rich begins job as telemarketer at Universal Case Company, working for commission
12/03/96 - Rich looses job at Universal Case Company
12/21/96 - Rich calls Tim about loosing his job, schedules a meeting
12/31/96 - Tim receives report from Medallion that Rich's services are complete
01/09/97 - Appointment cancelled because of bad weather, rescheduled for 1/28
01/28/97 - Rich cancels appointment because it is too cold. rescheduled
02/11/97 - Rich does not show up for appointment with Tim. rescheduled
02/25/97 - Appointment cancelled because Tim is sick. rescheduled
03/13/97 - Rich does not show up for appointment with Tim. rescheduled
04/02/97 - Rich & Tim meet. Rich only wants part-time work and does not want further job readiness training. They decide on Work Activities Evaluation at Medallion.
04/15/97 - ROC - Tim calls Rich to remind him to set up WAE
05/15/97 - Tim sends note to Rich reminding him that Rich needs to call Medallion about WAE
07/12/97 - Rich calls Tim to set up an appointment. scheduled for 7/30
07/30/97 - Rich cancels appointment. rescheduled for 8/21
08/21/97 - Rich cancels appointment. rescheduled
09/11/97 - Rich does not show up for appointment
09/26/97 - Rich's Caseworker calls Tim, says Rich is still interested. Appointment scheduled
10/16/97 - Caseworker can't find Rich to bring him to appointment.
