Counseling & Guidance were provided, financial assistance was provided to pay for tuition and fees

List the services which were provided and explain how these services enabled the client to secure or retain suitable employment:

 Ms. McKee received the following services through this agency: Guidance and counseling, books and supplies, and assistance with completing her master's degree. Ms. McKee has been receiving SSDI and disability assistance from the State. She was able to complete the degree requirements and now has obtained her MSW. She has been employed on a full time basis since May of 1998 and has made a satisfactory adjustment. All services have been completed and no further services are planned or indicated at this time. The client is satisfied with her employment situation as is her employer.

II. Client has achieved and maintained a suitable employment goal for at least sixty (60) days.

I HEREBY CERTIFY THAT THIS CLIENT IS "SUITABLY EMPLOYED" AS DEFINED BY THE FOLLOWING STATEMENTS:

• Client and employer are each satisfied.
• The client is maintaining adequate interpersonal relationships and acceptable behavior in the job environment.
• The occupation is consistent with the client's capacities and abilities considering the client's choice.
• The client possesses acceptable skills to perform or continue the work satisfactorily.
• The employment and working conditions will not aggravate the client's disability and the client's disability in the job situation will not jeopardize the health and safety of himself/herself or others.
• The employment is regular, reasonably permanent, and the client receives a wage commensurate with that paid others for similar work under legal requirements.
