Closure Statement for Karen's Case


After VR has helped a client find a job in a career of the client's choice, his or her case is officially closed. Below is the form Jack placed in Karen's casefile to document closing her file.

CLIENT NAME: Karen Spencer

 

I. What substantial services were provided an how did these services have an impact on client's employment?

Counseling & Guidance were provided, financial assistance for training, maintenance, and transportation were provided, job development/placement was provided.

List the services which were provided and explain how these services enabled the client to secure or retain suitable employment:

Karen was self-referred to Vocational Rehabilitation. This client was provided thorough diagnostic services, mostly through Medallion Enterprises, Inc. but also through Dr. Garner at the Medical Group in Clayton. Karen's evaluation was done at Medallion. It was determined after a thorough vocational evaluation at Medallion, that Karen would take part in the laboratory technician training. During this time, the client was provided financial assistance for transportation and personal maintenance, in addition to all training fees. Additional maintenance and transportation was provided for this client to take part in Job Club and Job Readiness Training at Medallion. She was also provided monies to purchase appropriate lab tech attire.

All in all, it is felt that the services and experiences that were made possible to this client through Vocational Rehabilitation from the time her case was opened until the present has enabled her to become quite successfully employed and independent and she has maintained this satisfactory employment for the past 60 days.

II. Client has achieved and maintained a suitable employment goal for at least sixty (60) days.
