March 10th, 1996 - transferred to University Hospital.
May 17th, 1996 - Contacted VR, self referral, for general help.
May 25th, 1996 - Released from University Hospital Rehabilitation Center, stays with his sister outside University City.
June 10th, 1996 - Moves into apartment in University City.
June 18th, 1996 - Meets Jack Greene, applies for VR.
July 22th, 1996 - Meets with Jack, found eligible for VR.
November 19th, 1996 - Contacts VR, decided to move back to Texas, case closed (status 30).
September 10th, 1996 - Begins volunteering at University Rehabilitation Center.
May 22nd, 1997 - Contacts VR, decided to stay, asked to have case reopened. Applies for part time opening at Independent Living Center.
June 4th, 1997 - Meets Jack Greene to discuss plans, reopen case.
June 12th, 1997 - Begins working at ILC part time.
July 8th, 1997 - Takes GED exam.
July 15th, 1997 - Meets with Jack, does Self Directed Search, G&C IWRP, agrees begin admissions to University and meet with advisors.
August 1st, 1997 - Begins working at ILC full time.
August 10, 1997 - Phone call to Jack - did not pass GED. No progress on application to University. College plans put on hold because working full time. Case closed status 28.
May 1 9, 1998 - ILC reduces Juan back to part-time because of budget problems. Requests a meeting to reopen his case.
June 12, 1998 - Meets with Jack to reopen case.
June 28, 1998 - Takes GED.
July 10, 1998 - Meeting with case worker for info interview.
July 15, 1998 - Meeting with Hank Baker at accessibility office.
July 20, 1998 - Applies to the University.
July 20 1998 - Phone conversation with Jack. He has passed GED.
July 24, 1998 - Meeting with academic advisor.
August 6th, 1998 - Meeting with Jack - review status, ready to enroll.
August 24th, 1998 - Starts class at University - Math 10, Spanish 135, and Intro to psych.
September 10th, 1998 - Phone conversation with Jack - things are going well.
October 20th, 1998 - Phone conversation with Jack- things are going well.
December 18th, 1998 - Meeting with Jack. First semester complete, waiting on grades.
January 5th, 1999 - begin 2nd training IWRP.
January 23rd, 1999 - Begins second semester.
February 18th, 1999 - phone call with Jack,
busy, but doing well.
